Responsibilities:
The service coordinator is responsible in ensuring that the needs of the residents are met by providing them with assistance and access to the required agencies and services in the community. They will provide informal counseling, referral program, plan educational programs, coordinate volunteer opportunities, link with outside service agencies and negotiate affordable services as needed. The service coordinator also educates residents on services, develops contacts with service providers and agencies for resident referrals and monitors provision of services. They work in conjunction with the manager(s) and other management staff of the property to empower residents to remain independent and self – reliant as possible. In addition, the service coordinator must have an I can do attitude, promote teamwork and positivity.
· Refer residents for emergency needs ie, rental assistance, food, medical, transportation
· Establish and maintain positive relationships with coworker, residents, and community agencies
· Promote wellness activities for all residents
· Provide educational opportunities
· Educate residents, families, and staff on available community resources
· Work as a team with property manager and other property staff
· Distribute yearly resident survey
· Maintain individual files on residents which may contain intake information, referral history, service provider history
· Maintain resident confidentiality
· Provide referrals to state, local and community agencies to alleviate resident need
· Assist with the bi-monthly newsletter
· Empower residents to meet their own needs through education and training.
Qualifications:
Background or education in social work, human services, or public health
· Excellent communication skills, both oral and written.
· Ability to maintain confidentiality
· Experience with community organizing
· Sensitivity and ability to relate to people from diverse lifestyles and cultures
· Good organization
· Strong interpersonal communication skills
· Team player
· Works well with other and independently
· Knowledge of eligibility for for federal and state entitlement programs
· Experience in planning community events and activities
· Knowledge of community resources
· Conflict resolution/Problem solving skills
